Directions

From Boston: Take I-93 North to Exit 33 (Winchester/Highway 28). The exit ramp goes into a rotary. Go almost 3/4 of the way around - look for South Border Road on the right. Follow South Border Road (Middlesex Fells reservation is on the right) to the intersection of Highland Avenue. Go straight through the intersection (Mt. Vernon Street). Continue on Church Street through Winchester until it intersects with Route 3. Turn right on Route 3 leading into Woburn. Rehabilitation Way is the first right immediately after Horn Pond Plaza. Country Club Heights is on the left-hand side at the end of Rehabilitation Way.

From Route 128: Take Exit 33A (Route 3 Woburn) off Highway 128. Stay on Route 3 (Cambridge Street) for approximately 4 miles. Go straight through a 5-way intersection (Woburn 4-Corners), pass the 99 Restaurant (on left), then enter a residential section. Country Club Heights Professional Building will be on your left. Rehabilitation Way is immediately after the professional building. A left-hand turn cannot be made here. Continue straight until you come to left access lane for Horn Pond Plaza. Make a u-turn in order to approach Rehabilitation Way from the South and take your first right. Country Club Heights is on the left-hand side at the end of Rehabilitation Way.
